Transplant Surgery
Transplant surgery involves organ procurement, preservation, implantation, and vascular anastomosis for kidney, liver, heart, lung, pancreas, and other solid organ transplants. CSTs manage complex vascular instrumentation, cold storage solutions, back tables, and multi-team coordination. Cases require precise vascular control, organ perfusion management, and meticulous sterile technique during extended operative times.
Role of CSTs
- Prepare back tables for organ reception with vascular instruments and anastomosis supplies
- Manage organ preservation solutions, cold storage, and perfusion systems
- Support vascular reconstruction with micro-instruments and tying platforms
- Coordinate multiple sterile fields for procurement and implantation teams
- Handle vascular grafts, staplers, and anastomosis reinforcement materials
- Maintain organ temperature control and viability monitoring equipment
- Assist with vascular access closure and hemostasis during reperfusion
Essential Skills
- Proficiency with microvascular and vascular anastomosis instruments
- Knowledge of organ preservation solutions and cold ischemia times
- Understanding of vascular reconstruction techniques and graft preparation
- Safe handling of back table sterile technique and organ preparation
- Coordination of multi-team sterile field management
- Familiarity with vascular stapling systems and tying platforms
- Management of anticoagulation and reperfusion protocols
Common Procedures
- Kidney transplantation (deceased and living donor)
- Liver transplantation (orthotopic and split liver)
- Heart transplantation
- Lung transplantation (single and double)
- Pancreas transplantation
- Kidney-pancreas combined transplant

Workflow & Case Progress
- Prepare back table with vascular instruments, tying platforms, and graft materials
- Set up organ preservation systems and cold storage monitoring
- Support recipient preparation with vascular clamps and dissection tools
- Manage organ reception, inspection, and back table preparation
- Assist with vascular anastomoses using micro-instruments and loupe magnification
- Coordinate reperfusion sequence and vascular flow assessment

Sterile Technique Considerations
- Back tables require dedicated sterile zones separate from main field
- Organ containers must maintain sterile integrity during transfer
- Vascular grafts demand glove change before handling
- Multiple team coordination increases cross-contamination risk
- Cold storage basins create condensation—manage drape integrity
- Extended case times require fresh sterile goods and glove changes
